A new program supporting you to take control of your health
Salveo Healthcare, in partnership with St Lukes, has launched the Health Reset Program pilot to offer extra support for people living with long-term health conditions. The program works alongside existing care, helping people feel more confident managing their health day to day.
Since the pilot began, people of many ages have shown interest. Participants can choose how they take part. Options include face‑to‑face visits, phone calls, or online appointments. This flexibility makes it easier for people to fit the program around their health, work, and daily life.
What is the Health Reset Program?
The program focuses on what matters most to each person. Over 12 weeks, participants meet one-on-one with experienced nurses to discuss health concerns, set goals, and create a simple plan. Additional support from a dietitian or physiotherapist is available based on individual needs.
The Health Reset Program looks at the whole person. It recognises that living with a long‑term condition can be challenging. This may include physical symptoms, low confidence, or trouble staying motivated. Early feedback shows that having time to talk, clear guidance, and regular support can make a real difference.
One participant, Katy, described how her nurse supported her with regular contact and guidance.
“It was the ability to be able to access a dietician that I wouldn’t have accessed otherwise to the same extent,” she said.
Katy found the program easy to fit around work and other commitments, and the app was easy to use.
As a pilot, the Health Reset Program is focused on learning what works best for participants. Insights will help guide future development.
